Patient History:
A patient presented with tooth 36 exhibiting a chronic periapical abscess with an associated fistula.

Diagnosis & Treatment Plan:
Due to the chronic infection and poor prognosis, extraction of tooth 36 was indicated, followed by delayed implant placement.

Clinical Procedures

Tooth Extraction:

  • Tooth 36 was atraumatically extracted.

  • The site was allowed to heal for one month before implant placement.


Implant Placement:

  • A DSI Premium Spiral implant was inserted at the edentulous site.

  • A cover screw was placed to protect the implant.

  • Primary closure was achieved with sutures.


Second Stage Surgery (after 2 months):

  • The cover screw was replaced with a healing screw WP 3 mm.

  • Uneventful healing was observed.


Digital Impression (after 1 week):

  • A DSI titanium scan body was used for a digital impression.


Prosthetic Phase (after 4 weeks):

  • A PFM crown was fabricated on a custom abutment.

  • The crown was screw-retained, with the access opening positioned occlusally.


Follow-Up & Outcome

1-Year Postoperative Control (X-ray & Clinical Examination):

  • Radiograph demonstrated successful osseointegration of the implant.

  • No bleeding on probing.

  • No deep periodontal pockets (>4 mm).

  • No signs of suppuration.

  • The crown was stable and functionally integrated.


Conclusion:

The treatment outcome demonstrates successful surgical and prosthetic rehabilitation of tooth 36 with a DSI Premium Spiral implant and screw-retained PFM crown. The case highlights predictable osseointegration, stable soft tissue response, and favorable long-term function.
