Implant Case Through Impacted Tooth

Lady, 52 years old, who had worn a denture for about 16 years and wanted a permanent solution. She had been advised she had to do extensive surgery to remove impacted tooth. She didn’t want the option. As she had seen the effect of surgery on her family members. So she opted to remain with denture. She visited our facility. Surgery and Ortho extrusion was ruled out. Various articles cite success of implants driven through impacted teeth if morbidity due to surgery is to be avoided. And have reported various degrees of success. So we opted for this technique.

Osteotomy prep through impacted tooth. Kindly note that high speed handpiece and straight slow speed with diamond tip was used to prepare the osteotomy through impacted tooth.

Implant drill for confirmation of through and through access through impacted tooth. Pulp was barely touched. Patient reported no pain after.

Implant torqued with high torque value of 80Ncm.

Post-op Implant with temp in situ. It’s also important to note that a 3.75 by 16mm length implant was chosen so as to engage the apical 5-7 mm of bone to offer additional volume for osseointegration.

Temp crown fabrication Using resin composite (DSI UniLite Nano).

Temp torqued to 35Ncm and gingiva sutured both palatal and buccal.
